| Our records indicate that you have not completed this year's CCPS Forms for your child(ren). These forms are available online and paper copies were also sent home with your child(ren). If you recently completed the forms online or submitted them to your child's school, please disregard this notice.

The video below walks you through the steps to complete the forms online. You may also complete the paper forms provided by your child's school and return them as soon as possible. It is important that CCPS has the most up-to-date information as possible in case of an emergency. There are four forms to complete:
- Emergency Information Form - Student Information
- Emergency Information Form - Health Information
- Deviation of Name Form
- Media Center Consent Form

- Click here to access CCPS Portal
- Log in as a parent (do not log in using your student's information)
- Click on the FOCUS icon in the Parent/Guardian section
- Follow the on-screen prompts to complete each form
